McNadoes Hardship Review Application

Complete all fields. Incomplete submissions are automatically rejected without review. All documentation requirements are mandatory. There are no exceptions to the documentation requirements, including the exception you are already thinking of.

Section 1 — Applicant Identification

Required for cross-reference with FEMA hardship registry.

Found on your most recent property tax statement. If you do not have this, you cannot submit this form.

Section 2 — Nature of Hardship

Minimum 250 words required.

Must be within the past 18 months. Hardships older than 18 months are considered pre-existing conditions and do not qualify for emergency review.

Section 3 — Prior Attempts with Conventional Services

You must document contact with at least three (3) conventional service providers who were unable or unwilling to address your hardship. McNadoes does not review applications from parties who have not first exhausted conventional options.

Section 4 — Atmospheric Impact Pre-Assessment

Applicants are required to complete a basic atmospheric impact pre-assessment prior to submission. This is not something McNadoes performs on your behalf at this stage.

Note: Applications without a licensed meteorologist's viability assessment are reviewed at lower priority. "Lower priority" on a hardship queue means additional delay.

Available through USDA Web Soil Survey at websoilsurvey.nrcs.usda.gov. If you are unsure what this means, consult a licensed soil scientist before submitting.

Section 5 — Declarations & Attestations

Before submitting, confirm that you have completed all required fields, attached all required documentation, and that you have not left any field blank with the intention of "coming back to it." The form does not save. If you close this window, you start over.
